Erectile Dysfunction

Patient Information Guide: Erectile Dysfunction (ED)

Introduction

Erectile dysfunction (ED) is a common condition that affects many men at some point in their lives. While it can be distressing, it is often treatable, and many options are available to help improve symptoms. This guide provides an overview of ED, including its incidence, symptoms, causes, investigations, and treatments, to support you in understanding the condition and taking the first steps towards effective management.

Incidence of Erectile Dysfunction

  • ED affects approximately 1 in 5 men over the age of 40.
  • The likelihood increases with age, with 50% of men over 50 and 70% of men over 70 experiencing some degree of ED.
  • Younger men can also experience ED, often linked to stress, lifestyle factors, or underlying health conditions.

What Is Erectile Dysfunction?

ED is defined as the inability to achieve or maintain an erection sufficient for satisfactory sexual performance. While occasional difficulties are normal, persistent issues may indicate ED.

Symptoms:

  • Trouble achieving an erection.
  • Difficulty maintaining an erection during sexual activity.
  • Reduced sexual desire or libido.

Possible Causes of ED

ED can result from a combination of physical, psychological, and lifestyle factors:

Physical Causes:

  • Cardiovascular disease: Reduced blood flow due to narrowed or blocked arteries.
  • Diabetes: Can damage nerves and blood vessels critical for erections.
  • Hormonal imbalances: Low testosterone levels.
  • Neurological disorders: Conditions like multiple sclerosis or Parkinson’s disease.
  • Prostate cancer and its treatments: Surgery or radiation can impact erectile function.
  • Medications: Side effects of certain drugs, including blood pressure medications and antidepressants.

Psychological Causes:

  • Stress, anxiety, or depression.
  • Performance anxiety.
  • Relationship difficulties.

Lifestyle Factors:

  • Smoking, excessive alcohol use, or recreational drugs.
  • Obesity and lack of physical activity.

Investigations for Erectile Dysfunction

To identify the underlying cause of ED, a thorough evaluation is conducted:

Medical History:

  • Questions about your symptoms, overall health, and any medications you are taking.
  • Discussion of psychological and lifestyle factors that may contribute to ED.

Physical Examination:

  • Assessment of the penis and testicles to check for abnormalities.
  • Evaluation of blood pressure, weight, and other signs of cardiovascular health.

Blood Tests:

  • To measure testosterone levels, PSA,  blood sugar, and cholesterol.
  • Screening for conditions like diabetes or hormonal imbalances.

Specialized Tests (if needed):

  • Penile Doppler Ultrasound: Assesses blood flow to the penis.
  • Nocturnal Penile Tumescence (NPT) Test: Checks for erections during sleep to determine if the cause is physical or psychological.
  • Psychological Assessment: For stress, anxiety, or other emotional factors.

Treatment Options for Erectile Dysfunction

The treatment approach depends on the underlying cause of ED and may include lifestyle changes, medications, and advanced therapies.

Lifestyle Changes:

  • Quit smoking: Smoking damages blood vessels, reducing blood flow to the penis.
  • Limit alcohol: Excessive drinking can impair erectile function.
  • Exercise regularly: Improves blood circulation and overall health.
  • Manage stress: Techniques like mindfulness or counseling can address psychological causes.

Medications:

  • Oral Tablets:
    • Sildenafil (Viagra), Tadalafil (Cialis), Vardenafil (Levitra): These phosphodiesterase-5 (PDE5) inhibitors enhance blood flow to the penis, helping to achieve and maintain an erection when stimulated.
    • Taken before sexual activity, with Tadalafil offering a daily low-dose option.
    • Side effects: Headache, flushing, nasal congestion, and upset stomach.
  • Topical Creams:
    • Alprostadil cream can be applied directly to the penis to enhance blood flow.
    • Provides an alternative for men who cannot take oral medications.
  • Penile Injections:
    • Alprostadil may be injected into the side of the penis near its base to cause an erection within 5-10 minutes.
    • Highly effective, particularly for men who do not respond to oral medications.
    • Side effects: Mild discomfort at the injection site or prolonged erections.

Devices and Surgical Options:

  • Vacuum Erection Devices:
    • A mechanical pump creates a vacuum to draw blood into the penis, followed by placing a constriction ring to maintain the erection.
  • Penile Implants:
    • Inflatable or semi-rigid devices surgically placed inside the penis.
    • Reserved for severe cases where other treatments are ineffective.
